When you leave the hands-on care of a hospital or facility, it is natural for some questions to arise:

• Am I taking the right medications?
• Has my condition changed?
• Should I be concerned about a new problem?
• Did I make it home okay?
• Do I have all the services I need?
• Is my house equipped properly?
• How do I move forward with my daily life?

The answers to these questions can determine how long your recovery takes, and how well you can recover. These questions are best answered by your primary care provider, but the last thing you and your doctor want you to be doing the day after discharge is getting in a car, driving to an office, and sitting in a waiting room.

Our team of medical providers are here to help! One of our providers will come to your home, review your history, perform a full examination, and talk to you about any and all questions you might have. 

• All of our providers are licensed to examine, diagnose, and treat any condition you may have. We can prescribe new medication, order and review in-home tests, and authorize new services. 
• We work WITH your doctor. We value your existing relationship, and will do everything possible to get you back under your doctor's care as soon as possible. We will see you as many times as you feel we need to, and then help move you back to your physician. We will also involve your existing physician in every aspect of our care.
